Exam Acommodation Procedures

Exam Accommodation Assisstance

When it is not possible for you to respond to a student's request for an altered testing format, location, or extended time, and your department cannot assist you, Disability Services can provide a proctored alternative site for testing.

Send Test Packet to Disability Services

Complete sections A, B and C of the Exam Proctoring Form and send it with the exam to Disability Services at least one week in advance via

Receive Completed Test Packet 

The completed exam is returned as specified in the Exam Proctoring Form within 24 hours.
