Print blank form on one sheet (front and back) and then fill out the COMPLAINANT INFO section only prior to submission to the University Police Department. $5.00 fee per key required at time of submission.
Access Requests:
Requests must be completed prior to submitting. All information requested must be included. Submissions will not be considered complete without all requested information, especially assigned UHCL ID#. Incomplete requests will be returned to the issuing authority.
PAN Codes
Print (both sides on one sheet, front and back), requestor must sign, and complete boxes I, and II. Box III must be signed by their instructor. Box IV must be signed by the Dean/Director or authorized designate.Key Requests
Prior to submitting this form to the Police Department, ensure that you have a current work assignment posted in PeopleSoft or have a pink slip from Human Resources to verify employment at the time of request. Upon receipt of a completed key request, the key(s) will be issued within 16 business hours.
