IMPORTANT NOTICE
Concerning the Immigration and Control Act of 1986
The University of Houston-Clear Lake complies with the Immigration Reform Control Act of 1986. Any individual hired after November 6, 1986 must present acceptable original proof of identity and authorization to work upon becoming employed, as specified by the Act. Only one document from List A is required. If List B and List C are used, one document from each list is needed to satisfy both the identity and employment eligibility requirement.
Original Proof Required – No Photocopies Will Be Accepted!
Do Not Begin Working Until I-9 Documentation Is Completed!
Failure to Document May Result In A Loss Of Wages!
LIST A - Documents that establish both identity and employment eligibility…
- U.S. Passport (unexpired or expired)
- Unexpired Foreign Passport, with I-551 stamp
INS Form I-94 indicating unexpired employment authorization - Permanent Resident Card or Alien Registration Receipt Card with photograph (INS Form I-551)
- Unexpired Temporary Resident Card (INS Form I-688)
- Unexpired Employment Authorization Card that contains a photograph (INS Form I-766, I-688, I-688A, I-688B)
- An unexpired foreign passport with an unexpired Arrival-Departure Record, Form I-94, bearing the same name as the passport and containing an endorsement of the alien's nonimmigrant status, if that status authorizes the alien to work for the employer
LIST B - Documents that establish identity only…
- State Driver’s License or ID Card with photograph or information such as name, date of birth, gender, height, eye color and address
- Other ID Card issued by federal, state or local government agencie with photograph
- School ID Card with photograph
- Voter’s Registration Card
- U. S. Military card or draft record
- Military dependent's card
- Driver's license issued by a Canadian government authority
LIST C - Documents that establish employment eligibility only…
- U.S. Social Security Card issued by the Social Security Administration (other than a card stating it is not valid for employment)
- Certification of Birth Abroad issued by the Department of State (Form FS-545 or Form DS-1350)
- Original or certified copy of a birth certificate issued by a state, county, municipal authority or outlying possession of the United States bearing an official seal
- Native American Tribal Document
- U. S. Citizen ID Card (form I-197)
