Exam Accommodation Assisstance

When it is not possible for you to respond to a student's request for an altered testing format, location, or extended time, and your department cannot assist you, Disability Services can provide a proctored alternative site for testing.

Send Test Packet to Disability Services

Create a test packet with the exam and Exam Proctoring Form with Sections A, B, and C completed. Send your test packet to Disability Services at least one week in advance via

• E-mail
• Hand-Delivery
  Students can deliver tests in a sealed envelopes.  
• Fax 

Receive Completed Test Packet 

The completed exam is returned as specified in the Exam Proctoring Form within 24 hours.
